CHRONIC (ABDOMINOPELVIC) PAIN AND DYSFUNCTION – WHEN STRESS AND ANXIETY MAKE IT WORSE – A CORINA AVNI MASTERCLASS

Outline
Thoughts and beliefs are real things – measurable electrical impulses in the brain resulting in hormonal responses – they have an impact on a variety of ‘background’ functions e.g. sleeping, blood pressure, breathing and digestion. We find ourselves in unprecedented times – whilst pandemics have come and gone, hardly any have occurred in a digital age attracting the attention of social media (Ebola and Zika being obvious exceptions). This has resulted in a ‘shadow pandemic’ of misinformation, fear and anxiety with many scientists and healthcare professionals predicting a second wave of COVID related mental-health and associated co-morbidities.

In this Masterclass we will deconstruct the impact of stress and anxiety on autonomic nervous system function. Special mention will be made of adverse childhood experiences; the advent on nervous system dysregulation. No Masterclass on chronic pain and dysfunction would be complete without addressing post traumatic stress disorder (PTSD) and endometriosis.

Specific populations will be identified, including but not limited to i. irritative bladder symptoms (frequency, urgency, pain and night-time voiding) ii. abdominal pain, IBS and constipation and iii. chronic pelvic pain. Life events that contribute to chronic pain and dysfunction will be discussed, and we will examine how pathology elsewhere impacts on abdominopelvic function – especially for those treating constipation by ‘palpating psoas’. And as if that is not enough, we will also make mention of diet, obesity, chronic inflammation and the gut microbiome! As always, we will remain culturally relevant and sensitive, whilst identifying vulnerable populations.

 to appreciate the multi-systemic experience that results in normal function
 to describe the likely presentation of 4 different autonomic nervous system dysregulations
 to identify anxious patients with multi-systemic autonomic co-morbidity
 to explain how co-morbidities can impact on anxiety
 to understand cultural, religious and psycho-emotive impacts of chronic abdominopelvic pain and dysfunction
